Latest in RO-ILS

  • Case Study 07 discusses an event in which confounding factors contributed to an error in which previous spinal radiation was not accounted for until a fortuitous conversation between physicians.
  • Case Study 06 discusses a relatively simple error of insufficient imaging during set up that could have significant impact on patient care.
  • Safety Notice regarding SRS heterogeneity corrections has been released. A RO-ILS Safety Notice communicates findings that may be novel or of higher clinical significance and necessitate prompt review by the radiation oncology community.

Since its launch in June 2014, more than 550 facilities across the country have joined RO-ILS: Radiation Oncology Incident Learning System® to contribute patient safety data to a national database. The mission of RO-ILS is to facilitate safer and higher quality care in radiation oncology by providing a mechanism for shared learning in a secure and non-punitive environment. Participation in RO-ILS is free thanks to the generous financial support of ASTRO, AAPM, Varian, ASRT, AAMD and Sun Nuclear Corporation. RO-ILS provides U.S.-based practices access to a secure, web-based portal and the ability to send data to a federally listed Patient Safety Organization (PSO). To learn more, watch an Introduction to RO-ILS video and contact RO-ILS staff.
